Scabies has been a problem for humanity for centuries. Cyclical epidemics have occurred in many Western countries every 15 or 30 years. Scabies is caused by a mite that burrows under the skin and causes a contagious infection that spreads rapidly between family members, close personal contacts and within congested communities. If all affected are not treated adequately, the problem will persist. Resistance in the parasite can also lead to prolonging of the problem.

Scabies can be a marker disease for patients whose immune system has been compromised. The incubation period is about three weeks, but re-infestation occurs more quickly. The higher the number of parasites on a person, the higher the contagiousness of the human host. The mite, its feces and the eggs burrowed under the skin cause irritation, itching and often secondary infections caused by scratching. The skin may show small insect-type bites on it, or the lesions may look like pimples. The rash can be subtle, and sometimes scabies is hard to diagnose. Scabies frequently occurs in the crevasses of the body. Symptoms include severe and continuous itching, which can be more noticeable at night. Crusted or Norwegian scabies is the most potent form of Scabies. In crusted scabies, the body is covered with a crusty rash. The rash of crusted scabies may not itch, but crusted scabies is the most contagious form of scabies and the hardest to treat. It often requires multiple treatments, sometimes with more than one medicine.

Approved and Off label Treatments

Permethrin in a cream prescription strength formula is a popular remedy. It is both powerful in effect with the least toxicity. It is at least as effective as Lindane, that can be dangerous when misapplied. It is more potent than the approved Crotamiton or Sulfur treatments.

Permethrin kills the parasite in any stage of its life cycle. This medicine should be applied and left on the body for over 10 hours. It needs time to penetrate where it can reach the parasite. The symptoms can last for longer as they are a reaction to the embedded mites.

Lindane is another FDA approved drug; but, it is not recommended for babies, women who are pregnant or breast feeding, older adults, anyone who weighs less than 110 lbs, has a seizure disorder, people who have a weakened immune system or have skin conditions. Overuse, misuse, or accidental swallowing can be toxic. Lindane is only used in persistent cases or if the patient is unable to use other less risky treatments. The oral medication Ivermectin may be prescribed off label in the U. S. For cases that are difficult to treat with other medication, or in patients with immune deficiency, or patients with skin conditions that should not use topical medications. Since 1993, use in some countries has found success in treating scabies resistant to other treatment. The advantage of using Ivermectin is that it is given orally as a single dose. It has also been used as a public health measure to control widespread scabies in population groups. Ivermectin is not used in small children or in women who are pregnant or breastfeeding.

Crotamiton is FDA approved for use in adults. Benzyl benzoate is allowed for use in Europe. It can be a cheaper treatment. Sulfur is not not FDA approved ;But, has been prescribed for patients who cannot have a stronger medicine.

If symptoms continue this could be the result of a reaction to the medication, although more likely explanation is improper use of the prescribed medicine or inadequate treatment of the close contacts of the patient, within the household or within a congested community. Antihistamine may be used to control the symptoms of itching, whereas skin infection treatment may involve the prescription of antibiotics. Steroid creams and in bad cases of itching, steroid pills may be taken. Should there be adverse reactions, the affected person should seek medical attention.
